SCIBA HS Jazz Band Registration
Please be reminded that members MUST pay SCIBA dues as well as IBA dues to participate in SCIBA events. The dues are $10 annually.
The first and second place winners in each class qualify for the Iowa Jazz Championships
Five separate classes: 4A, 3A, 2A, 1A, and Second Band
Plaques in each class
Outstanding Player and Section Awards
Three adjudicators will give each band written and taped comments
Clinic following your performance
Class 2A, 2nd Band, and 4A held at Waukee High School
Class 1A and 3A at Norwalk

To submit additional bands:
- Submit this form for your band using the appropriate class (1A, 2A, etc.)
- After submitting it use your browsers BACK arrow to come back to the form (all of your director information should still be there)
- Change the CLASS field to "2nd Band"
- Submit the form again for each additional band
- You should receive a confirmation email for each submission.
Completing your registration:
- A copy of this form will automatically be emailed to Wayne Page at wpage@waukee.k12.ia.us
- Please ALSO print a hard copy of this form and mail to Cheryl Pittman with your full payment at Ballard H.S., P.O. Box 307, 701 Ballard Dr., Huxley, IA 50124
Entry fee is $125 per band if postmarked by October 15
